Sacrament Certificate Requests

Parishioners may request certificates for Baptism, First Communion, Confirmation, Marriage or Death from Our Lady Queen of Peace church by filling out the Sacrament Certificate Request form below or by calling 337-233-1591. There is an administrative fee of $3 per certificate, due upon time of pick up. If you are unable to pick up the certificate in person, please call us to make other arrangements.
